, an Assisted Living Facility in the Eden Senior Care Network, is looking for a Resident Care Lead
. The resident care lead is responsible for maintaining the resident's medical records and assuring the smooth functioning of the unit, in addition to providing care. The Resident Care Lead is also responsible for maintaining staff schedules accurately.
Resident Care Lead Shift
Schedule:
PM shift 3 pm-11 pm Essential Functions
- Maintains a positive and professional demeanor toward residents, visitors, families and co-workers.
- Works closely with nursing staff, physicians and residents families in order to provide the best care for the resident. This includes, but is not limited to, reporting resident concerns to the MD/family/care manager, reporting incidents to applicable parties, documenting on incidents/changes of condition, requesting orders.
- Assist with updating care assignments for residents as needed d/t changes in condition or increased needs.
- Completes incident reports as needed.
- Enters orders into MAR/PCC as needed. Follows up on medications not in the facility to ensure they are ordered. (includes diabetic supplies)
- Provides supervision to caregivers. Works with DOW to provide training and corrective counseling as needed.
- Maintains confidentiality of resident information.
- Monitor call light system for extended call lights times and notify DOW of concerns to follow up on.
- Performs medication cart audits, at a minimum, of monthly at the direction of the DOW.
- Develops and maintains staff schedules.
- Attends all required training, in-service and staff meetings.
- Follows company uniform and grooming standards.
- Maintains a safe working environment.
- Adheres to all policies and procedures of Eden Senior Care.
- On - call requirement every other week and weekend.
- Coordinate and manage care services for individuals in need.
- Develop and implement care plans based on individual needs and goals.
- Collaborate with healthcare professionals, families, and other stakeholders to ensure comprehensive care.
- Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
- Provide direct support to individuals, including assistance with daily activities and personal care.
- Develop and maintain accurate staff schedules, filling all open positions timely. Monitor and track attendance for all care team members.
- Assist with the interview and hiring process, including orientation.
- Previous experience as a caregiver is required.
- Familiarity with systems for managing care services is a plus.
- Previous experience with staff scheduling.
- Ability to lift and assist individuals with mobility needs.
- Strong communication and interpersonal skills.
- Excellent organizational and time management abilities.
