Position: RN / Registered Nurse Case Manager - Hospice - $5,000 SIGN ON BONUS

RN / Registered Nurse Case Manager – Hospice

Sign‑On Bonus $5,000

Full‑time, located at 325 N Corporate Dr, Suite 260, Brookfield, WI 53045. Requires travel primarily throughout Waukesha and Milwaukee counties.

Schedule:

Monday‑Friday, 8 AM – 5 PM.

  • Lower patient‑to‑nurse ratios for meaningful, unrushed hospice care
  • Strong interdisciplinary team support—collaborative care with Social Work, Chaplaincy, Aides, and providers
  • Leadership that understands hospice nursing
  • Dedicated internal after‑hours triage team to support nurses outside of regular business hours
  • Homecare Homebase (HCHB) EMR for streamlined documentation

Responsibilities
  • Assess/monitor physical, emotional, and psychological needs of patients
  • Create hospice care plans that align with the patient’s wishes and goals
  • Direct nursing care: administer medications, treatments, and interventions
  • Provide pain and symptom management
  • Educate and support the patient’s family and caregivers
  • Collaborate with interdisciplinary team
  • Maintain accurate and timely documentation
  • Participate in on‑call rotation as required by local branch
Qualifications
  • Graduate of an accredited nursing school with current licensure to practice in the state of operation
  • One year nursing experience required; 3+ years preferred
  • Experience in hospice or a similar setting preferred
  • Knowledge of the hospice philosophy of care
  • Commitment to clinical and documentation excellence
  • Must be a licensed driver with an automobile that is insured and in good working order
