Hospice RN Case Manager RN

Location:

168 West 22730 Prairie View Lane, Jackson, WI 53037

Coverage area: Travel throughout the territory required - primarily Washington, Ozaukee, and northeast Milwaukee counties

Schedule: Full Time, Monday-Friday 8AM-5PM

• Assess and monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient’s wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year nursing experience required; 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
