This weekend-only role offers the opportunity to make a meaningful difference in patient recovery, dignity, and quality of life.
You’ll work closely with an interdisciplinary care team in a supportive environment designed to help providers focus on what matters most—exceptional patient care.

What You'll Do- Provide primary care services for adult and geriatric patients with acute and chronic medical conditions.
- Evaluate patients, establish diagnoses, prescribe medications, and perform diagnostic and therapeutic procedures within scope of practice.
- Develop individualized treatment plans in collaboration with physicians and interdisciplinary care teams.
- Coordinate care with specialty providers, including wound care, psychiatry, and other consulting services.
- Communicate changes in patient condition and treatment plans with patients, families, facility staff, and healthcare team members.
- Maintain timely, accurate, and compliant documentation.
- Build strong relationships with patients, families, and facility staff to support exceptional care.
- Perform other duties as assigned.
- Current Nurse Practitioner or Physician Assistant license.
- Active DEA registration.
- BLS certification.
- Strong clinical assessment and diagnostic skills.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Commitment to compassionate, patient-centered care.
- Leadership experience or demonstrated leadership skills.
- Strong organizational and time-management abilities.
- Positive, solution-oriented attitude.
- Experience working with adult and geriatric populations.
