Commonheart is seeking a Hospice Patient Care Coordinator (PCC) to provide clinical leadership and mentoring for our hospice team in Wisconsin. You will supervise nursing staff, coordinate patient care plans, and support the interdisciplinary team to ensure high-quality, patient-centered care.
Requirements include WI RN license, 4–5 years hospice experience, and a valid WI driver’s license.
Hours:
Mon–Fri 8:00–4:30 with rotating on-call. Salary: $88,400–$105,352 yearly plus benefits.
