Job Description:

Join our compassionate healthcare team as a Rehabilitation Registered Nurse (Rehab RN), where your expertise and dedication will be valued in helping patients achieve their highest level of function and independence. In this vital role, you will collaborate with interdisciplinary teams to provide personalized care, support recovery, and improve quality of life for individuals in a nurturing, dynamic environment.

Responsibilities:
  • Conduct comprehensive patient assessments to determine rehabilitation needs.
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans accordingly to achieve rehabilitation goals.
  • Educate patients and their families about treatment plans and self-care techniques
  • Collaborate with physical therapists, occupational therapists, and other rehabilitation professionals to optimize patient outcomes
  • Document patient care activities accurately and maintain up-to-date medical records
  • Ensure compliance with health and safety regulations and protocols
  • Assist in the discharge planning process to ensure continuity of care post-rehabilitation
  • Stay updated on best practices and advancements in rehabilitation nursing care
Requirements/

Qualifications:
  • Valid and current Registered Nurse (RN) license in the state of practice
  • Bachelor of Science in Nursing (BSN) preferred
  • Minimum of 2 years of nursing experience in a rehabilitation or related setting
  • Certification in Rehabilitation Nursing (CRRN) preferred or willingness to obtain
  • Basic Life Support (BLS) certification required
  • Advanced Cardiovascular Life Support (ACLS) certification preferred
  • Strong knowledge of rehabilitation nursing principles and practices
  • Proficiency in electronic medical records (EMR) systems
  • Excellent communication and interpersonal skills, Strong problem-solving and critical thinking skills
  • Ability to work independently and as part of a multidisciplinary team
  • Experience Entry Level (0-2 years)
