WI-Waupun Memorial Hospital – Patient Care Worker (PRN)

• Schedule is 7a.m. – 3p.m.
• Minimum of 2 shifts per month required.
• Monitor telemetry and check vitals when patients exercise in the gym.
• ACLS certification required.

Provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.

• Implements and monitors patient care plans.
• Monitors, records, and communicates patient condition as appropriate.
• Serves as the primary coordinator of all disciplines for well‑coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates patient discharge planning needs with the healthcare team.
• Applies evidence‑based practice to deliver patient‑centered nursing care.
• Uses the ANA Code of Ethics for Nurses to guide responses to evolving health needs.
• Maintains constant alertness and ensures a safe work environment.
• Performs other duties as assigned.

Graduate of an accredited school of nursing or equivalent educational background for licensing.

One year of registered nurse experience.

• Constant standing and walking.
• Frequent lifting, carrying, and pushing/pulling objects weighing 0–25lbs.
• Frequent sitting, reaching, and keyboard use/data entry.
• Frequent visual, hearing, and olfactory use for patient care.
• Occasional lifting, carrying, and pushing/pulling objects weighing 25–50lbs.
• Occasional patient movement and repetitive motions.
• Occasional driving.
• Rare crawling and running.

• Illinois – BLS HCP (American Heart Association) and Registered Professional Nurse (RN) issued by Illinois Department of Financial and Professional Regulation.
• Missouri – BLS HCP (American Heart Association) and Registered Nurse (RN) issued by Missouri Division of Professional Registration.
• Oklahoma – BLS HCP (American Heart Association) and Registered Nurse (RN) issued by Compact State; or RN issued by Oklahoma Board of Nursing.
• Wisconsin – BLS HCP (American Heart Association) and Registered Nurse (RN) issued by Compact State; or RN issued by Wisconsin Department of Safety and Professional Services.
