• Prepare and administer medications and perform specialized treatments adhering strictly to the Wisconsin scope of practice.
• Conduct resident assessments, identify status changes, and collaborate with RNs for proactive intervention.
• Maintain precise, real-time electronic health records (EHR) to ensure regulatory compliance.
• Supervise and mentor Certified Nursing Assistants (CNAs) to ensure personal care is delivered with high dignity.

• Licensure:
  Current, unrestricted LPN license in Wisconsin.
• Skills:
  
  Exceptional clinical judgment, strong organization, and proficiency with electronic charting systems.
• Experience:
  
  Previous long-term care experience beneficial; new graduates welcome with orientation program.
