Responsibilities
  • Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms
  • Complete F2F encounters for hospice patients as needed
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Prescribe medications and coordinate care for patients with advanced or chronic illnesses
  • Accurately document all encounters in the EMR
  • Communicate effectively with patients, families, and other healthcare professionals
Qualifications
  • Experience preferred but willing to train new Nurse Practitioners!
  • Active Nurse Practitioner license in Wisconsin
  • Strong clinical assessment, documentation, and care planning skills preferred
  • Experience with eClinical

    Works (eCW) EMR preferred (willing to train)
  • Excellent communication and teamwork abilities
