- Mileage Reimbursement
- Conduct comprehensive palliative care visits, including assessment, diagnosis, and management of symptoms
- Complete F2F encounters for hospice patients as needed
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Prescribe medications and coordinate care for patients with advanced or chronic illnesses
- Accurately document all encounters in the EMR
- Communicate effectively with patients, families, and other healthcare professionals
- Experience preferred but willing to train new Nurse Practitioners!
- Active Nurse Practitioner license in Wisconsin
- Strong clinical assessment, documentation, and care planning skills preferred
- Experience with eClinical
Works (eCW) EMR preferred (willing to train) - Excellent communication and teamwork abilities
