Summary / Objective

Under the general supervision of a Registered Nurse and reporting to the Executive Director, the Licensed Practical Nurse (LPN) provides direct nursing care and clinical oversight for assigned residents/tenants in accordance with Wisconsin DHS 83 and/or DHS 89 regulations. This role supports the health, safety, and well‑being of residents by monitoring medical conditions, administering medications and treatments, documenting care, and collaborating with interdisciplinary staff to deliver individualized, person‑centered services.

The LPN promotes resident dignity and independence while ensuring adherence to facility policies, state regulations, and nursing standards of practice.

Responsibilities
  • Coordinate and review clinical referrals in a timely manner with center leadership or liaisons in the field; support the nursing manager in completing the full admissions process, including patient chart review, diagnosis, orders, hospital EMR, laboratory and imaging results.
  • Coordinate admission decisions per the clinical capabilities grid and center operations.
  • Assist central intake coordinators with clinical decision making in specific circumstances.
  • Identify clinical opportunities per referral types and enter assigned tasks into EMR.
  • Respond to admission inquiries promptly and follow the North Shore Admission Acceptance/Denial Process.
  • Coordinate execution of conversion steps with the center to improve conversion ratio.
  • Maintain strong working relationships with center leadership and liaisons.
  • Utilize the PCC CRM database to enter all referrals and outcomes.
  • Provide 24/7 admission coverage support with central office team members.
  • Adhere to North Shore sales philosophy and all policies and procedures.
  • Comply with all applicable laws, regulations, and the Center’s Compliance Program.
  • Follow center rules, dress and hygiene policies, smoking policies, and report any incidents, hazardous conditions, or equipment needs.
  • Use proper protective equipment and follow infection control standards.
  • Know and protect resident rights; report complaints and maintain confidentiality of resident information; adhere to HIPAA requirements.

Required

Education and Experience
  • Current Wisconsin LPN license in good standing.
  • Experience in assisted living, long‑term care, or a related setting preferred.
  • Strong communication, organizational, and leadership skills.
  • Ability to work collaboratively with the RN and other healthcare professionals.

Education and Experience

Post‑acute care or health care experience preferred. Experience in a fast‑paced environment.
