Registered Nurse (RN) Community Treatment Opportunity:

The Registered Nurse (Community Treatment) provides comprehensive nursing services to individuals receiving community-based behavioral health services through the Assertive Community Treatment (ACT) program. The nurse works collaboratively within a multidisciplinary team including psychiatrists, therapists, service facilitators, and community partners—to support client stability, recovery, and overall health. Services are delivered in the home and community settings and include medication management, administration of long-acting injectable medications, physical health monitoring, symptom assessment, client education, and care coordination.

  • The starting pay range for this role is $36.40 - $39.63, based on experience.
  • This job is a full-time, in person position, usually working weekday business hours.

What You Bring:

Required:
  • Current Wisconsin Registered Nurse license
  • Must possess a valid driver's license and maintain unrestricted driving privileges throughout employment.
  • Must meet and continue to meet all requirements for insurability under the organization's vehicle insurance policy.
  • Must be authorized to operate any vehicle and transport clients, patients, or staff as required by the position.
  • Current BLS certification
  • More than 1 year of experience in healthcare, customer service, or a closely related field.
  • Strong interpersonal and communication skills when working with clients, families, and multidisciplinary teams.
Preferred:
  • Bachelor’s degree in Nursing (BSN)
  • More than 1 year of experience in psychiatric, behavioral health or community based nursing.

What a Registered Nurse (Community Treatment) does at NCHC:
  • Provide nursing services to individuals enrolled in Community Treatment programs in accordance with professional nursing standards and organizational policies.
  • Conduct comprehensive assessments of clients’ mental and physical health status, including symptom evaluation, vital signs monitoring, and health screenings.
  • Administer medications, including long-acting injectable medications, ensuring adherence to the 'five rights' of medication administration.
  • Monitor medication effectiveness, side effects, and client compliance; communicate concerns and recommendations to prescribing providers.
  • Provide education to clients regarding medication, disease management, wellness, treatment adherence, and recovery-oriented practices.
  • Conduct routine symptom assessments to evaluate cognition, mood, behavior, functioning, and ability to perform daily living activities.
  • Monitor and assess physical health conditions including blood pressure, blood glucose, weight, pain levels, and other health indicators.
  • Coordinate care and communicate regularly with multidisciplinary team members including psychiatrists, therapists, service facilitators, case managers, and external providers.
  • Advocate for clients during medical appointments and coordinate follow-up care to support treatment adherence and continuity of care.
  • Respond to clients experiencing psychiatric or behavioral crises by utilizing therapeutic de-escalation techniques and coordinating appropriate services when necessary.
  • Document assessments, interventions, medication administration, and client interactions in the electronic health record in accordance with agency policies and regulatory standards.
  • Participate in interdisciplinary team meetings, clinical supervision, and service planning to support client treatment goals.
  • Maintain accurate records of medication inventory, supplies, and disposal in accordance with organizational policies and regulatory requirements.
  • Travel within the community to conduct home visits, client meetings, and outreach services.
  • Maintain required licensure, certifications, and continuing education necessary to perform nursing duties.
  • Follow ACT program principles including person-centered care, recovery orientation, trauma-informed practice, and strength-based support.
  • Perform other duties as assigned to support program operations and client care.
