Licensed Registered Nurse in Wisconsin employed to identify, provide, and accept responsibility for the fulfillment of patient’s total nursing needs. This includes assessment, medication management, documentation, supervision of other nursing personnel, and acting as care manager.

• Responsible for on-call services as assigned.
• Participates as part of the core team providing care and services; coordinates implementation of the plan of care.
• Actively participates in coordination of all aspects of patient’s care in accordance with current professional standards and practice.
• Advocates for patient choice.
• Participates in developing and revising written patient care policies and procedures.
• Actively participates in home health quality assurance and performance improvement program.
• Provides education and counseling to the patient and to the patient’s family, consistent with the plan of care.
• Actively participates in home health-sponsored in-service training and continuing education opportunities.
• Completes all documentation within time frame determined by policy, including ongoing observation and assessment of patient and family needs.
• Leads, guides, and directs others in the provision of quality care.
• Practices infection control mitigation at all times.
• Maintains honest, open, and professional communication with others, including promoting the essence of the interdisciplinary team.
• Responds by phone to patient and family calls in a timely manner.
• Performs other duties as assigned.

• Must be currently licensed as a Registered Nurse in Wisconsin.
• At least 2 years of experience in Medical Surgical, Home Health, Hospice, or Long-Term care nursing preferred.
• Certified in Gerontology, or Wound Care preferred.

• Monday–Friday: 8:00 AM to 4:30 PM.
• 0.6 FTE (24 hrs/week), 0.8 FTE (32 hrs/week), or 1.0 FTE (40 hrs/week).
• Rotating on-calls.

Role includes travel within a one-hour radius. Company car provided for employees working 0.6 FTE or more.

• $37.54 to $46.58 per hour, based on experience. Higher pay may be considered depending on qualifications and experience.
