We're currently seeking a full-time, hourly, Home Health Field Clinician who evaluates new clients for eligibility, completes OASIS assessments, obtains consents, and develops individualized care plans.

• Educate clients and families on Home Health services
• Communicate findings with physicians and care team
• Ensure timely and accurate documentation
• Participate in care coordination, scheduling, and on-call rotations
• Collaborate with an interdisciplinary team to ensure high-quality, coordinated care in clients’ homes

• Current Wisconsin RN license (BSN preferred)
• 1+ year of Home Health RN experience
• 1+ year of OASIS experience
• Strong communication and assessment skills
• Valid Wisconsin driver’s license, reliable transportation, and required insurance coverage
