Hospice LPN

About

the Role

St. Camillus is seeking a compassionate and dependable Hospice LPN to provide skilled nursing support to hospice patients under the direction of an RN Case Manager or Hospice Manager. In this role, you will help deliver individualized, comfort-focused care while supporting patients' physical needs, emotional well-being, and dignity.

What You'll Do
  • Provide nursing care to hospice patients in accordance with each patient's plan of care and under RN supervision.
  • Accurately perform and document vital signs, observations, visits, and telephone communications in a timely manner.
  • Communicate changes in condition, symptoms, and patient needs to the RN Case Manager or Hospice Manager.
  • Support interventions that promote comfort, safety, dignity, and quality of life for patients and families.
  • Follow all St. Camillus policies, safety procedures, confidentiality standards, and regulatory requirements.
  • Participate in care coordination, team communication, quality assurance activities, required training, and mandatory meetings.
  • Provide reliable, flexible support based on patient and family needs, including on-call coverage as required.
What We're Looking For
  • Current Wisconsin LPN license.
  • Minimum of one year of medical-surgical experience as an LPN.
  • Hospice or home care experience preferred.
  • Current CPR/BLS certification.
  • Strong communication, documentation, and customer service skills.
  • Comfort using computers, electronic documentation, email, internet navigation, and basic files or electronic documents.
  • Ability to safely perform essential job functions with or without reasonable accommodation.
  • Valid driver's license, current vehicle liability insurance, and a driving record that complies with the St. Camillus Safe Driving Policy when using a personal vehicle.
