Working as part of a multidisciplinary team, the Case Manager plays a critical role in reducing barriers to care and promoting client stability, recovery, and access to resources. This position requires a high level of organization, collaboration, and commitment to client-centered, culturally responsive services.
Primary Responsibilities Conduct assessments to identify client needs across multiple domains (e.g., housing, safety, medical, legal, financial)
Collaborate with clients and clinical staff to develop individualized service plans
Monitor and update client needs and goals throughout the course of services
Provide ongoing case management services to support client stability and recovery
Link clients to needed resources and services, including shelter, housing, healthcare, legal advocacy, and public benefits
Assist clients with applications (e.g., victim compensation, benefits, insurance)
Coordinate care across multiple providers and systems
Conduct proactive and assertive outreach to engage and retain clients in services
Maintain regular contact with clients, including phone, in-person, and community-based interactions
Support re-engagement of clients who have disengaged from services
Advocate on behalf of clients within healthcare, legal, and social service systems
Provide trauma-informed support and psychoeducation
Assist clients in navigating complex systems and reducing barriers to care Participate in multidisciplinary team meetings and case conferences
Collaborate closely with clinicians, medical providers, and external partners
Communicate effectively with team members regarding client needs and progress
Maintain accurate, timely, and complete documentation in accordance with program and funding requirements
Track services provided, referrals, and client outcomes
Support data collection, reporting, and quality improvement efforts

May include accompanying clients to appointments (e.g., court, medical visits)
Requires flexibility to meet client needs, including occasional evening hours
Qualifications
Appropriate experience may be substituted for education on an equivalent basis.
Minimum Required
Education:
Associate’s degree
Minimum
Required Experience:
2 years
Preferred Education:
Bachelor’s degree in social work, psychology, public health, or a related field
Preferred Experience:
Minimum of 1-2 years of experience in case management, social services, or a related field
Bilingual/bicultural (e.g., Spanish/English)
Experience working with trauma-affected populations strongly preferred
Knowledge of community resources and public systems (e.g., housing, benefits, legal services)
Experience working with survivors of violent crime
Familiarity with trauma-informed care and culturally responsive practices
Physical Requirements Work requires occasionally lifting moderate weight materials, standing, or walking continuously.
Work Environment Occasional exposure to dust, noise, temperature changes, or contact with water or other liquids. Work is performed in an environmentally controlled environment.
Sensory Acuity Ability to detect and translate speech or other communication required. May occasionally require the ability to distinguish colors and perceive relative distances between objects.#LI-AV1
