About the RoleSt. Camillus Hospice is seeking a compassionate and dedicated Social Worker to provide psychosocial support, care planning, assessment, and guidance for clients receiving hospice services. In this role, you will help clients, families, and caregivers navigate end-of-life needs with dignity, comfort, and respect while partnering closely with pastoral care and the interdisciplinary hospice team.
What You’ll Do- Complete initial and ongoing social work assessments for hospice clients.
- Develop and support individualized plans of care that address social, emotional, economic, and family needs.
- Provide counseling, education, bereavement support, and community resource referrals for clients and families.
- Partner with the interdisciplinary team, including pastoral care, to coordinate compassionate end-of-life care.
- Maintain timely and accurate documentation in accordance with agency policies and regulatory requirements.
- Mentor and support other hospice team members while managing your caseload with professionalism and accountability.
- Master of Social Work (MSW) required.
- Current Wisconsin Social Worker License required.
- Hospice or related experience preferred.
- Strong communication, crisis-response, time-management, and collaboration skills.
- Ability to work independently, prioritize client needs, and travel to client homes as needed.
- Valid driver’s license, current vehicle liability insurance, and an acceptable driving record when using a personal vehicle.
