LPN Medical Care Manager

We are seeking an LPN Medication Nurse responsible for providing safe and accurate medication administration, health monitoring, and precise clinical documentation.

• Administering medications and treatments according to physician orders
• Following the Six Rights of Medication Administration
• Documenting in MAR/TAR accurately and in real time
• Monitoring symptoms and administering medications for relief
• Reporting any refusals or adverse reactions promptly
• Conducting Service & Health Updates (SHUs)
• Transcribing physician orders and supporting care plan updates
• Observing and reporting physical, emotional, or cognitive changes
• Maintaining medication carts, completing narcotic counts, and ensuring compliance standards

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred

This role emphasizes resident safety, regulatory adherence, and clinical precision to prevent medication errors and identify health changes early.
