LPN Med Care Manager

Role Overview: As an LPN Medication Nurse, you provide licensed nursing support by administering medications, monitoring residents’ health, and maintaining accurate clinical documentation.

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document accurately in MAR/TAR in real time
• Monitor symptoms, administer medications for relief, and report any adverse reactions or refusals
• Conduct Service & Health Updates (SHUs) and support care plan updates by transcribing physician orders
• Observe and report physical, emotional, or cognitive changes in residents
• Maintain medication carts, perform narcotic counts, and ensure compliance with safety standards

• CPR and First Aid certifications
• Strong assessment, observational, and documentation skills
• Experience in senior care or medication administration is preferred
