Position: Physical Therapist (PT) - PRN
  • Role: Physical Therapist (PT) - PRN

  • Schedule:
    Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed)

  • Location: Field Based

  • Compensation (Productivity Points): $75.00 / point

We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives
  • Carry a high-quality caseload

  • Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

  • Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

  • Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

  • What you'll own
    • Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits.

    • Complete comprehensive in-home assessments and deliver skilled PT visits.

    • Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention.

    • Set measurable goals, track objective progress, and adjust treatment quickly when patients stall.

    • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

    • Manage treatment and outcomes directly, including PTA supervision where appropriate.

    • Coach patients and caregivers with practical home programs that actually get done.

    • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.

    Who you are / how you operate
    • Dedicated clinician: home health experience strongly preferred

    • High ownership: you treat your caseload like your own panel and your pod like your own business.

    Must-haves
    • Active Michigan PT license (in good standing)

    • Strong clinical judgment and comfort working independently in the field

    • Comfortable escalating changes in condition to RN/MD and documenting rationale clearly

    • Strong communication and documentation

    • Reliable follow-through on orders, frequencies, and patient goals

    • Current driver’s license and comfort with daily travel across assigned territory

    • Able to carry/transport basic therapy equipment as needed

    • Competency in core home health PT scope

      • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

      • Strengthening, functional mobility, and home safety assessment/modification education

      • Patient/caregiver coaching with practical home programs

    Nice-to-haves
    • Prior home health experience

    • Homecare Homebase (HCHB) experience (or similar home health EMR)

    • Deeper clinical experience in high-signal PT populations

    • Experience supervising/mentoring PTAs and collaborating on care plans

    • Strong organization and time management

    • $1k sign-on bonus paid after first 90 days
