As a Home Health Physical Therapist, you will evaluate, develop, and implement individualized treatment plans that improve mobility, reduce pain, restore function, and promote independence. This role requires travel throughout Wayne and Monroe Counties.
Key Responsibilities- Perform comprehensive physical therapy evaluations.
- Develop individualized treatment plans.
- Provide therapeutic exercises, gait training, balance training, transfer training, and fall prevention education.
- Recommend and train patients in the use of assistive devices and adaptive equipment.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on home exercise programs and safety techniques.
- Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals.
- Complete accurate and timely electronic documentation.
- Current Michigan Physical Therapist license.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
- Current auto insurance.
- Excellent communication and clinical assessment skills.
- Ability to work independently.
- Home Health experience.
- Medicare documentation experience.
- Experience using Electronic Medical Record (EMR) systems.
Flexible PRN scheduling. Available opportunities include weekdays, weekends, and additional visits based on patient needs.
