As a Home Health Occupational Therapist, you will evaluate patients and provide individualized occupational therapy services designed to improve their ability to perform activities of daily living (ADLs), increase independence, and maximize safety within the home.
This position requires travel throughout Wayne and Monroe Counties.
Key Responsibilities Patient Evaluation & Treatment- Perform comprehensive occupational therapy evaluations.
- Develop individualized treatment plans.
- Provide interventions to improve activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Assess and recommend adaptive equipment and home modifications.
- Improve upper extremity function, coordination, strength, and fine motor skills.
- Educate patients and caregivers on energy conservation, safety, and adaptive techniques.
- Collaborate with physicians and the interdisciplinary healthcare team.
- Complete accurate and timely electronic documentation.
- Current Michigan Occupational Therapist license.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
- Current auto insurance.
- Strong communication and patient education skills.
- Ability to work independently.
- Home Health experience.
- Medicare documentation experience.
- Experience using Electronic Medical Record (EMR) systems.
Flexible PRN scheduling.
Available opportunities include weekdays, weekends, and additional visits based on patient needs.
