The Community Health Worker position centers on helping individuals enrolled in Behavioral Health Home (BHH) services improve their overall health and well-being through personalized support and care coordination. The role involves building supportive relationships with clients, encouraging engagement in their care plans, and helping them achieve health-related goals. A significant part of the job is assisting individuals in navigating both behavioral health and physical health systems, particularly those managing complex co-occurring mental and physical health conditions.

The Community Health Worker works closely with the Behavioral Health Home care team and collaborates across departments within and outside of the organization to ensure services are delivered in a coordinated, person-centered manner. This includes collaborating with healthcare providers and other team members to address clients’ needs, remove barriers to care, and connect individuals with appropriate resources and services. The position also emphasizes mentoring and empowering participants to take a more active role in managing their health, promoting better access to care, increased engagement in treatment, and improved health outcomes.

Overall, the role combines direct client support, advocacy, care coordination, and health system navigation to help individuals successfully manage their health and recovery.

QUALIFICATIONS AND REQUIREMENTS Education

High school diploma or equivalent required. Community Health Worker certification at the time of hire is preferred. If not currently certified, successful completion of a Community Health Worker certification is required as a condition of employment following hire. Must complete all required annual compliance trainings in accordance with Behavioral Health Home standards, as well as applicable state, federal, and organizational policies and procedures.

Experience

Minimum of one year of experience working with individuals with disabilities required. An associate degree or higher in a human services or healthcare-related field may be considered in lieu of required work experience. Experience coordinating healthcare appointments, services, or other relevant human services or healthcare-related roles is preferred. Candidates should demonstrate the ability to work independently with a high level of integrity and ethical responsibility.

Proficiency in computer use is required. Essential functions of this position include frequent typing, data entry, and the ability to cut, paste, and manage electronic information. This role requires regular use of a mouse or keypad, as well as navigation within electronic health record systems, including moving between screens, utilizing tabs and links, and applying electronic signatures. Additional computer-related responsibilities include working with Adobe PDF, scanning documents, and maintaining accurate electronic records and files.

Frequent and consistent computer use is an essential function of this position.

Travel

Adjusted hours and extensive scheduled and non-scheduled travel is required in executing essential duties and responsibilities of this position. Employees must provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle registration and insurance in compliance with State of Michigan requirements are all continuing conditions of employment. If driving a vehicle other than the one listed on file with the Human Resources Department, employee is responsible for ensuring the vehicle is registered, insured, and meets all other State of Michigan motor vehicle requirements and providing documentation as requested.

CLS policy restricts the use of an alternate driver without the express, prior written authorization of the Human Resources Director. A review of such request will be based on evaluation of HIPAA compliance issues, work restrictions, other relevant factors, and whether or not the request will present an undue hardship to the organization.
