    Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed) Minimum of 10 points per week

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives
  • Carry a high-quality caseload:
    Minimum of 10 productivity points/week at target visit quality.

  • Own your patients  outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

  • Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

  • Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

  • What you ll own
    • Own outcomes, not just visits: you are accountable for the patient’s SLP plan, progression, and functional outcomes.

    • Complete comprehensive in-home SLP evaluations and deliver skilled SLP visits.

    • Assess and treat (as ordered/clinically indicated):

      • Aphasia and other communication disorders

      • Cognitive-communication impairment (attention, memory, executive function) impacting safety and independence

      • Dysphagia screening/assessment and swallow safety education (within home health scope and orders)

    • Build and execute a clear plan of care with measurable, functional goals.

    • Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving.

    • Coach patients/caregivers with practical strategies and routines that actually get done.

    • Document promptly and thoroughly — defensible, efficient, and consistent.

    Requirements
    • Active Michigan ST license (in good standing)

    • Reliable transportation, valid driver s license, auto insurance

    • Home health experience strongly preferred

    • Able to carry/transport basic therapy equipment as needed

    • Competency in core home health PTA scope

      • Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations

      • Strengthening, functional mobility, and home safety assessment/modification education

      • Patient/caregiver coaching with practical home programs
