BHI helps connect all three to allow doctors to extend care beyond the office, provider staff to centralize management for greater efficiency, and patients to receive TDD right in their own home with experienced nurses.

BHI helps bring physicians, provider staff, and patients together for Better Home Infusion.

Basic Home Infusion is hiring for a Patient Care Coordinator to join our team in Wayne, NJ. This is a full-time opportunity that works Monday-Friday in office.

Essential Job Functions:

  • Tracking Admission Packets: scanning and saving to patients’ charts and maintaining Admission tracking XLS and Entering information into
  • Maintaining Medication Profiles in Care Tend System: adding, changing & deleting medication in patients’ profiles. On average, we receive 20 emails daily with changes
  • Care Plan tracking:
    Faxing care plans to MDO for signature, maintaining Care Plan Spreadsheets, scanning and adding to patient charts in Care Tend) The daily average for this task is 25 to fax out.
  • Confirm RN Visits:
    Auditing the visits, making sure Telemetry matches with RN Notes and if there are discrepancies send back to RN to correct, entering future Refill and/or Interim visits as well as any necessary plotted dates. If any dates change in or out of the Pharmacy window, Pharmacy, Pharmacy Tech and Pharmacy Alert all need to be notified. After confirming each visit, Telemetries and RN Notes are faxed to the MDO and saved in Care Tend
  • Call Center patient maintenance:
    Changing any visit requests from RNs , Assigning different RN’s to patients and notifying Pharmacy of changes, Requesting Telemetries from MDO, Save PTM (Personal Therapy Manager) pictures to patient charts, entering patient alerts, allergies, address changes, MD changes, contacting MDO if new to obtain NPI#, DEA# & License#

In addition to the above daily tasks, at the end of each month review and investigate files to track down all missing visits from RN’s, review PN’s, seek any existing compounding records, confirm that visits were generated by RN Express and if not send missing visit emails to RNs.

Qualifications

  • High school degree or GED
  • Proficient in Microsoft Office (primarily Word and Excel)
  • Ability to multitask
  • Must be detail oriented
  • Strong oral and written communication skills
  • Bachelor’s Degree preferred
  • 1 year medical office experience preferred
  • Ability to sit for extended periods of time
