Holistic are Hospice is hiring a Full-Time Compassionate Admissions/Resource Nurse (RN) to provide patient care in and around the Wayne Area. $5,000 SIGN-ON BONUS.

- Hands-on Clinical Onboarding Program to ensure a smooth transition into our team
- Review clinical referral information and respond to requests and inquiries as appropriate. Collaborate with Clinical Director regarding the eligibility and appropriateness of clients for hospice services.
- Provide information, pre-assessment, and admission services to patients who have been referred to the hospice; conduct pre-assessment visits in the home or healthcare facility as necessary.
- Communicate with the Intake Department, the referring physician and the Hospice Medical Director to assure appropriateness of each referral and that the necessary certifications are obtained.
- Apply nursing principles and techniques in the treatment of patients. Provide nursing care, emotional support and education to patients and families. Encourage patients and family members to participate in the patient’s plan of care to meet established goals.
- Participate in On-Call duties as assigned and defined by Clinical Director and Program Director.
- Bachelor's degree in nursing or health care preferred.
- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
- Hospice/home health experience preferred but not required.
- Must be computer proficient in typing and various programs, including background in EMR.
- Possess and maintain current CPR certification.
- Current driver's license, vehicle insurance, and access to dependable transportation.
$85,000 - $93,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
