You'll address the physical, psychosocial, and spiritual needs of patients and families to improve function, independence, and quality of life.

Position:
Weekend Inpatient Hospice Nurse Liaison/Case Manager
Shift: Day Shift;
Three out of Four Weekends per Month;
Saturday & Sunday
Experience:
1. Two Years recent med-surg experience in an acute or long-term care setting, rehabilitation, home care, or hospice setting preferred.
Education:
RN Diploma, ASN required; BSN preferred.
Licensures/
Certifications:
1. Current license to practice nursing in PA OR a multi-state license verified in one of the approved states through the Nursing Licensure Compact.
2. Valid Driver's license and automobile insurance.
3. Provider BLS (American Heart Assoc/American Red Cross) required.
4. CHPN preferred.
