Holistic are Hospice is seeking a full-time Hospice RN Case Manager to join our nursing team delivering compassionate end-of-life care throughout the Delaware County area including Media, Wallingford, Broomall and Newtown Square.
You will coordinate services, empower patients and families, and collaborate with our Clinical Director while providing hands-on hospice nursing care in a supportive, growth-oriented environment with a comprehensive benefits package.
Schedule:
Mon–Fri, 8am–4:30pm.
