Per Diem​/On-Call LPN

Human Good community is seeking a Per Diem/On-Call LPN for our senior living community. In this flexible role, you'll provide direct nursing care to older adults, including medication administration, treatments, monitoring resident conditions, and responding to changes in health status. You'll collaborate with RNs, physicians, and caregiving staff, document in the EMR, and support care plans that promote dignity, safety, and independence.

Responsibilities

• Provide direct nursing care to senior living residents following care plans and physician orders
• Administer medications and treatments accurately and safely
• Monitor, document, and report changes in residents' conditions using EMR systems
• Collaborate with RNs, physicians, and interdisciplinary team members
• Support residents' dignity, safety, and independence in daily activities
• Respond promptly to resident needs, emergencies, and call systems
• Educate residents and families on health conditions and care instructions
• Follow infection control, safety, and regulatory standards
• Assist with admissions, discharges, and care transitions as needed
• Participate in team meetings and contribute to quality improvement efforts
