Job Overview
The Registered Nurse – Home Health will provide and direct nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. The RN will coordinate the total plan of care with other healthcare professionals, facilitate continuity of patient care through communication with physician, agency personnel, patient, family, and community resources.
All activities will be performed within a point-of-care setting.
- Provide nursing care to patients in their homes.
- Follow physician orders and comply with laws and agency policies.
- Coordinate patient care plan with other healthcare professionals.
- Maintain continuity of care by exchanging information with physician, agency personnel, patient, family, and community resources.
- Graduate from a recognized, accredited school of nursing.
- Minimum 1 year of clinical experience.
- Minimum 1 year of hospice experience.
- RN – Registered Nurse – Georgia State licensure and/or NLC/eNCL multistate licensure upon hire.
- CPR – Cardiac Pulmonary Resuscitation upon hire.
- Valid driver’s license and access to a dependable vehicle or public transportation upon hire.
