We are seeking a dedicated Licensed Practical Nurse (LPN) for a full-time Home Health role in Charlottesville, VA.
Responsibilities- Providing 1:1 patient care following individualized care plans while maintaining dignity and independence
- Administering medications, feedings, oxygen, and ostomy care, and assessing patient outcomes
- Documenting all nursing actions, communicating with family, case managers, and physicians, and updating on patient progress and condition changes
- Coordinating supplies, medications, and ensuring patient needs are met per physician instructions
- Licensed Practical Nurse or equivalent with current registration and license in the state
- Valid driver's license, acceptable driving record, and proof of insurance
- CPR certification
- Preferred: 1+ years of nursing and home health experience
patient-centered care, documentation accuracy, effective communication, ability to work independently, and team support.
