Summary:
The Home Care Registered Nurse Coordinator PACE plans, organizes, and implements in-home services for elderly participants, acting as a liaison between home health agencies and the PACE program. Responsibilities include coordinating daily operations, assessing participants in their homes, developing and managing care plans, and utilizing telehealth and community resources to promote health and wellness. The role involves participating in interdisciplinary teams, maintaining medical records, and ensuring compliance with competency requirements.
Qualifications & Skills: Requires a Bachelor of Science in Nursing or a commitment to obtain one within 6 years, current RN licensure, and a valid driver’s license with reliable transportation. Must have at least 2 years of experience working in interdisciplinary teams within hospital, nursing home, or community settings, and a minimum of 1 year working with frail or elderly populations.
Preferred skills include computer literacy, telehealth familiarity, and community-based nursing experience. Strong communication, assessment, and care coordination skills are essential.
Key Duties: Coordinate in-home services, assess participant needs, develop care plans, facilitate communication among care teams, utilize technology for health management, and ensure documentation accuracy. The role emphasizes compassionate, patient-centered care to support the health and independence of elderly individuals in their homes.
