Role:
The Home Care Registered Nurse Coordinator PACE plans, organizes, and implements in-home services for elderly participants, acting as a liaison between home health agencies and the PACE program. Responsibilities include assessing participants, coordinating care plans, managing interdisciplinary team collaboration, and utilizing telehealth and community resources to promote health and wellness in home environments.
Qualifications:Requires a Bachelor of Science in Nursing or an agreement to obtain it within 6 years, current RN licensure, valid driver’s license, and reliable transportation. Must have at least 2 years of experience in interdisciplinary teams, with 1 year working with frail or elderly populations. Preferred experience includes community-based nursing, EMR familiarity, and mental health or ACTT RN experience.
Skills & Abilities:Strong assessment, coordination, and communication skills, with the ability to work independently in patients’ homes and collaborate effectively with teams. Emphasizes compassionate patient care and adaptability to diverse home environments.
