Lifecare Home Health - - Responsibilities:
Treat the patient in accordance with a program designed by the occupational therapist and coordinate with team members;
Provide education and training to patients and family regarding therapy plan, home exercise program, and safety;
Take measurements related to patients condition and report changes to supervising OT;
Complete documentation of patient treatment and progress for the occupational therapist and agency;
Participate in face-to-face patient conferences and mandatory case conferences
