Lifecare Home Health - - Responsibilities:
Provide direct patient care to patient under the direction of the registered nurse and according to the Aide Plan of Care;
Assist in providing a safe transfers and ambulation per Hospice policy;
Provide necessary skills to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care;
Practice infection control principles and maintain a clean, safe environment;
Perform on-call/after-hours responsibilities and provide on-call/after-hours services to patients and families as assigned
