Responsibilities:- Provide direct patient care, including assistance with daily living activities, vital signs monitoring, and mobility exercises
- Develop and maintain effective relationships with residents, families, and healthcare teams
- Collaborate with licensed nurses to develop and implement individualized care plans
- Maintain accurate and detailed records of resident care
- Participate in quality improvement initiatives and contribute to a positive and respectful work environment
