We are actively hiring a Social Worker to support residents and families while helping ensure their emotional, social, and psychosocial needs are met. The Social Worker plays an important role in advocating for residents, coordinating services, supporting care planning, and helping residents navigate their stay. As a Social Worker, you will work closely with residents, families, nursing staff, and the interdisciplinary team to promote quality of life and positive resident outcomes.

Key Responsibilities
  • Provide social services and support to residents as a Social Worker
    , addressing individual emotional, social, and psychosocial needs
  • Advocate for residents and help promote dignity, choice, independence, and quality of life
  • Participate in care planning and collaborate with the interdisciplinary team to develop and support individualized resident goals
  • Communicate with residents, families, staff, and other involved professionals regarding resident needs and concerns
  • Assist with discharge planning, referrals, resources, and coordination of services as appropriate
  • Maintain accurate and timely documentation in accordance with facility policies and applicable regulations
Qualifications
  • Degree or certification in Social Work or a related field as required by state and facility regulations
  • Previous experience in a skilled nursing, long-term care, senior living, or healthcare setting preferred
  • Strong communication, organization, and interpersonal skills
  • Compassionate and resident-centered approach
  • Ability to work effectively with residents, families, and an interdisciplinary care team
  • Strong attention to detail and ability to maintain confidentiality
