• $3.50/hr weekdays 7pm‑7am, Monday‑Friday (shift ends Friday at 7am)
• $4.00/hr weekends 7pm Friday‑3am Monday

• Assist with activities of daily living for participants at the PACE center and in their homes.
• Contribute to development of care plans through interaction with the Interdisciplinary Team.
• Collaborate with the Interdisciplinary Team to ensure appropriate and continuous care.
• Carry out non‑skilled treatments including vital signs, transfers, toileting, bathing and dressing at the PACE Center and in the community.
• Assist supervisors and staff with individual and group programs, planning, motivation and evaluation of program effectiveness.
• Assist registered therapists with treatments and activities ordered by the therapist.
• Maintain a clean and safe environment in the PACE center and participants’ homes.
• Assist with meal and snack preparation, serving, feeding and clean‑up.
• Use safe techniques in all interactions with participants.
• Provide accurate and timely documentation in the EMR and other systems.
• Participate in daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines.
• Participate in distribution of work for health aides to ensure timely care and site task completion.
• Participate in regular team meetings for health aides.
• Perform all duties in accordance with FH and Summit Elder Care policies and procedures.

Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable; equivalent work experience is acceptable.

Certification: Certification as a Home Health Aide or Nursing Assistant is desired.

Reliable transportation required.

Experience: One year of experience working with frail and/or older adults, preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to independent work with participants. Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis.
