What You'll Do
- Evaluate and treat patients with communication, cognitive, or swallowing disorders
- Develop individualized care plans following all regulatory and clinical standards
- Conduct assessments using standardized tools and evidence-based practices
- Track patient progress and modify treatment plans as needed
- Evaluate treatment effectiveness and adjust interventions accordingly
- Provide counseling/support to patients, families, and caregivers
- Communicate regularly with the healthcare team regarding patient status
- Document all services thoroughly in electronic medical records
- Support supervision of CFYs or student SLPs when applicable
- Participate in team meetings, trainings, and conferences
What You Need
- Current ASHA and State licensure as an SLP
- Prior experience in long-term care preferred
- Strong clinical assessment and intervention skills
- Excellent communication and teamwork abilities
