Description:
Utilizes evidence-based practice to support clinical interventions.
Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with physical functioning disorders under the supervision and direction of a Physical Therapist.
Administers various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, and the use of supportive and assistive devices.
Assesses patient progress and communicates with the supervising Physical Therapist to adjust treatments accordingly in the clinical record.
Reports the effectiveness of treatment outcomes to the supervising Physical Therapist.
Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
Provides input to supervisors for discharge summaries of services provided.
Documents all required information in the electronic documentation system and maintains patient records according to company policy.
Accurately records treatment minutes for services provided as required.
Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings.
Participates in facility and required trainings, in-services, and conferences.
Works with supervising Physical Therapist to obtain necessary durable medical equipment or supplies per requirements.
Supervises and supports student under the guidance of the Physical Therapist.
Adheres to state required practice acts, code of conduct, and ethics during professional practice as a Physical Therapist Assistant.
