• Carry out physical therapy treatment plans developed by the Physical Therapist to address residents' mobility and functional needs
• Deliver direct therapy services, including therapeutic exercises, gait training, and balance activities, in a safe and effective manner
• Observe and report resident responses to interventions, including progress and changes in condition, to the supervising PT
• Assist with resident assessments and contribute to documentation and care planning as appropriate
• Educate residents and caregivers on exercises and techniques to promote safety and carryover outside therapy sessions
• Maintain accurate, timely, and compliant documentation in the electronic medical record system; ensure proper use and care of therapy equipment and assistive devices
• Participate in team meetings and collaborate with interdisciplinary staff to support coordinated resident care
• Adhere to all state, federal, and facility regulations and professional standards of practice; uphold licensure and continuing education requirements

• Graduate of an accredited Physical Therapist Assistant program
• Current PTA license in the state of practice (Massachusetts)
• Prior experience in SNF or geriatric setting preferred
• Strong communication, clinical judgment, and documentation skills
• Ability to work effectively as part of a rehabilitation team under the supervision of a licensed PT
