Job Summary and Qualifications
The clinical pharmacist shall practice the profession of pharmacy with utmost ethics, demonstrating the organization’s mission and values. The pharmacist is responsible for safe, effective, evidence‑based, and cost‑effective selection and utilization of medications for patients. Using advanced knowledge, skills, and professional judgment in all aspects of work, the pharmacist serves as a patient advocate within multidisciplinary rounding teams, collaborating with other health professionals.
Adherence to all regulatory and medication safety guidelines is paramount.
What you will do in this role:
- Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols
- Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervenes when orders are unclear, potentially unsafe, or inappropriate
- Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately and follows facility practices and procedures for documentation in the patient’s medical record
- Troubleshoots/clarifies Centralized Order Entry (COE) orders
- Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines
- Evaluates Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate
- Issues controlled substances to patient care areas and maintains records as required by law and follows corporate policies and procedures related to the dispensing and monitoring of controlled substances
- Ensures safe, appropriate, evidence‑based, and cost‑effective medication therapies for patients according to established policies, procedures, and protocols
- Monitors medication therapy regimens for contraindications, drug‑drug interactions, drug‑food interactions, drug‑laboratory test interactions, allergies, appropriateness of drug and dose, indications and therapeutic duplications
- Based on knowledge & expertise, develops, assists, implements, and executes all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy‑directed dosing
- Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed for drug‑bug mismatch, de‑escalation and duration of therapy
- Reads, extracts, and interprets information in patient charts accurately
- Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner
- Shares good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services
- Reviews medication related events that have occurred to prevent recurrences
- Models, mentors, and encourages pharmacy students, residents, and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity.
- Sustains the formulary by instituting drug therapy modifications for formulary compliance, SKU reduction and management of shortages as applicable
- Evaluates and responds to patient‑specific drug information questions from clinicians
