Job Summary and Qualifications

The clinical pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization's mission and values. The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work.

The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the practice of the clinical pharmacist.

What you will do in this role:

  • Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols
  • Verifies Computerized Physician Order Entry (CPOE) orders for accuracy and appropriateness and intervenes when orders are unclear, potentially unsafe, or inappropriate
  • Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately and follows facility practices and procedures for documentation in the patient's medical record
  • Trouble shoots/clarifies Centralized Order Entry (COE) orders
  • Prepares and dispenses medications, including sterile, chemotherapy, and parenteral nutrition preparations safely and accurately using appropriate techniques, USP 797 & 800 guidelines
  • Evaluates Total Parenteral Nutrition (TPN) for appropriateness based on facility policies and guidelines or ASPEN guidelines or recommendations as appropriate
  • Issues controlled substances to patient care areas and maintains records as required by law and follow corporate policies and procedures related to the dispensing and monitoring of controlled substances
  • Ensures safe, appropriate, evidence-based, and cost-effective medication therapies for patients according to established policies, procedures, and protocols
  • Monitors medication therapy regimens for contraindications, drug-drug interactions, drug-food interactions, drug laboratory test interactions, allergies, appropriateness of drug and dose, indications and therapeutic duplications
  • Based on knowledge & expertise, develops, assists, implements, and executes all drug therapy management protocols including, but not limited to; pharmacokinetics, anticoagulation dosing, IV to PO, pain management, antimicrobial stewardship, and pharmacy-directed dosing
  • Reviews/interprets culture and susceptibility data for antibiotic appropriateness and recommends changes, as needed for drug-bug mismatch, de-escalation and duration of therapy
  • Reads, extracts, and interprets information in patient charts accurately
  • Detects and reports suspected adverse drug reactions and medication errors accurately and in a timely manner
  • Shares good catches and clinical outcomes to pharmacy leadership to elevate pharmacist interventions and services
  • Reviews medication related events that have occurred to prevent recurrences
  • Models, mentors, and encourage pharmacy students, residents, and colleagues in a manner that fosters highly professional pharmaceutical care and high personal standards of integrity.
  • Sustains the formulary by instituting drug therapy modifications for formulary compliance, SKU reduction and management of shortages as applicable
  • Evaluates and responds to patient-specific drug information questions from clinicians
  • Provides drug information to patients and their families, performs applicable medication reconciliation
