Role Overview: A Home Health Physical Therapist provides patient-centered care in patients' homes, focusing on restoring mobility, independence, and safety. The role involves evaluating functional status, developing personalized care plans, delivering hands-on therapy, and educating patients and caregivers.
Key Responsibilities- Assess patients' mobility, strength, and rehabilitation potential
- Implement and adjust individualized treatment plans in collaboration with the care team
- Provide hands-on therapy to improve mobility, balance, and strength
- Monitor progress, support safe care transitions, and assist in discharge planning
- Partner with interdisciplinary team members and document care accurately
- Supervise Physical Therapist Assistants and support caregivers
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- 1-2 years of experience, preferably in home health
- Strong clinical assessment, critical thinking, and communication skills
- Current CPR certification, valid driver’s license, reliable transportation
