Join a compassionate home health team as a Physical Therapist, delivering patient-centered care in patients' homes to promote independence and recovery.
Responsibilities Include- Evaluating patients' functional status and developing personalized care plans
- Providing hands‑on therapy to improve mobility, strength, and balance
- Educating patients and caregivers on exercises and safety
- Monitoring progress, adjusting treatment, and supporting care transitions
- Collaborating with interdisciplinary teams and documenting care
- Supervising Physical Therapist Assistants and guiding caregivers
- Graduate of an accredited PT program with a current license
- 1–2 years of clinical experience, preferably in home health
- Strong assessment, communication, and critical thinking skills
- CPR certification, valid driver’s license, reliable transportation
Ideal candidates are autonomous, passionate about patient outcomes, build trusting relationships, and combine clinical expertise with compassionate care.
