Role Description This is a full-time, on-site Pharmacist in charge role located in Webster, TX. The Pharmacist in charge will verify, prepare, and dispense medications for hospice and long-term care patients in accordance with clinical protocols, regulatory requirements, and safety standards. Day-to-day responsibilities include reviewing prescriptions for appropriateness, checking for drug interactions, counseling care teams on medication usage, and documenting all activities in the pharmacy systems.

The Pharmacist will collaborate closely with nurses, physicians, and other healthcare professionals to support comfort-focused care plans and respond to urgent medication requests. Additional duties include maintaining accurate inventory, supporting quality assurance initiatives, and contributing to continuous process improvement in a fast-paced, patient-centered environment.

Qualifications

  • Valid PharmD or BS in Pharmacy from an accredited institution and active, unrestricted Texas pharmacist license
  • Experience or strong interest in hospice, palliative care, or long-term care pharmacy settings
  • Knowledge of clinical pharmacology, drug interactions, and medication therapy management for complex, multi-morbid patients
  • Knowledge of inventory management.
  • Ability to use pharmacy information systems and electronic health records, with attention to detail and accuracy in documentation
  • Strong communication and collaboration skills for working with interdisciplinary care teams and supporting family-centered care
  • Demonstrated commitment to compassionate, patient-centered service and adherence to ethical and regulatory standards.
  • Ability to work on-site in Webster, TX, manage time effectively, and perform in a high-volume, deadline-driven environment.
  • Prior experience in a long-term care or specialty pharmacy is preferred; board certification or additional clinical training is a plus
