Director Of Case Management

A community hospital in the Jersey City area is looking for a Director of Case Management to join them. This person will lead the case management department while supporting a small team with surgical patient cases, level-of-care decisions, discharge planning, and regulatory compliance.

• Provide day-to-day leadership, guidance, and support to Case Managers and Social Workers
• Review complex cases and help determine the most appropriate care plan, discharge path, or alternative level of care
• Serve as a clinical resource for utilization review, documentation, medical necessity, length of stay, and regulatory questions
• Complete higher-level reviews related to patient status and medical necessity when cases require additional review
• Partner with physicians, nursing, social work, administration, and interdisciplinary teams to address barriers to discharge and improve patient outcomes
• Participate in rounds, length‑of‑stay meetings, and utilization review committees to support efficient patient progression
• Educate physicians and clinical teams on documentation, level of care, discharge planning, and utilization management expectations
• Help ensure compliance with CMS, payer, state, federal, and accreditation requirements
• Support process improvement initiatives related to care coordination, electronic documentation, quality, and regulatory readiness
• Coordinate with community agencies and post‑acute resources to help patients transition safely after discharge

• Nursing Degree required
• Active NJ RN license
• 1+ year of nurse leadership experience
• Previous case management experience preferred
