Position Summary

The Advanced Practice Professional (APP) coordinates comprehensive cardiology patient care, performs diagnostic and therapeutic procedures, and collaborates with multidisciplinary teams. APPs provide education to residents, medical/nursing students, and staff.

Core Duties and Responsibilities
  • Manages comprehensive and individualized care for cardiology patients and their families in inpatient and immediate post-discharge ambulatory settings.
  • Completes daily rounds; consults and/or pre-procedure/evaluations.
  • Completes integrated assessments.
  • Orders additional tests or consults following established clinical guidelines and protocols.
  • Performs and communicates results of diagnostic tests within scope of practice.
  • Consults and communicates with clinic and hospital staff regarding abnormal findings.
  • Performs succinct comprehensive clinical evaluations and documents findings in the medical record.
  • Facilitates patient discharge by coordinating appropriate services and tests prior to discharge for outpatient follow-up appointments.
  • Develops individualized plans of care, including teaching, discharge planning, and referrals as necessary.
  • Demonstrates expertise in caring for patients across the age continuum.
  • Identifies unstable or complex cardiology patient situations and facilitates appropriate intervention.
  • Recognizes emergency situations and initiates effective emergency care.
  • Evaluates patients and serves as a coordinator for follow-up, procedures, and tests in the ambulatory setting.
  • Assists in the collection of outcome measures.
  • Demonstrates critical thinking and diagnostic reasoning in clinical decision making.
  • Utilizes equipment safely.
  • Bases decisions on relevant scientific principles, standards of care, and research findings.
  • Utilizes teaching theory and strategies for education of staff and patients.
  • Promotes a learning environment responsive to patient and family preferences.
  • Updates knowledge base throughout the year.
  • Completes all identified clinical competencies.
  • Serves as a resource for staff caring for cardiology patients.
  • Provides educational programs to professional and paraprofessional personnel.
  • Participates in community programs, literature distribution as appropriate.
  • Documents plans of care, interventions, and evaluations clearly and concisely.
  • Collaborates with other leaders and staff to improve patient outcomes.
  • Leads development and implementation of changes that positively impact outcomes.
  • Works collaboratively to facilitate problem solving and provide support.
  • Initiates interdisciplinary approaches to patient care, program development, and education.
  • Promotes innovation of expanded roles in inpatient and ambulatory delivery of care.
  • Provides leadership for committees and task forces.
  • Documents hours and submits billing reports as appropriate.
  • Analyzes cardiology data to recommend and implement changes.
  • Participates in clinical trials and research studies.
  • Assists faculty, administrators, and directors with outcome measure collection.
  • Other duties and projects as assigned.
Minimum Qualifications for Nurse Practitioner
  • Graduate of a CCNE accredited nursing program with a Master’s Degree in Nursing.
  • National certification by a recognized APRN certification center.
  • Licensure or eligibility for licensure as a professional Registered Nurse and Advanced Practice Registered Nurse in West Virginia.
  • BLS certification required; additional Advanced Life Support training as determined by the department.
Experience
  • Acute care or cardiology experience preferred.
  • Eligible new graduates will be considered.
Preferred Qualifications for Nurse Practitioner
  • ACNP preferred; FNPs will be considered.
Minimum Qualifications for Physician Assistant
  • Bachelor’s Degree as a Medical/Surgical Physician Assistant required.
