Job Summary

Case Manager – Full Time Day Shift at Wellington Regional Medical Center in Wellington, Florida. The hospital is a 235-bed acute care facility owned by Universal Health Services, Inc.

• Ensure appropriate utilization of hospital services.
• Develop discharge plans and coordinate services in a timely manner.
• Recognize signs and symptoms of stroke, initiate stroke alerts, and verbally communicate the process.
• Collaborate with stroke patients and families on discharge planning based on patient needs, condition, and prognosis.
• Maintain current stroke knowledge by completing required online training.

• Florida RN license.
• 2–4 years recent acute care experience.
• Experience in utilization chart review and discharge planning.
• Managed care experience preferred.
• Knowledge of Inter Qual Criteria.
• Strong clinical background and interpersonal communication skills.
• Basic computer skills.

• Reports to the Director of Case Management.
• Must be a Florida RN.
• 2-4 years recent acute care experience required.
• Experience in utilization chart review and discharge planning.
• Managed care experience preferred.
• Knowledge of Inter Qual Criteria.
• Strong clinical background needed with strong interpersonal communication skills.
• Basic computer skills.
