To provide brief, time-limited psychotherapy services to the patients of Confluence Health.
Position Reports To Practice Manager
Essential Functions- Conduct Intake Assessments for patients according to departmental procedures, funding source requirements, and evidence-based/strength-based model approaches.
- Develop Individualized Treatment Plans for each patient according to policies of the department and funding source requirements.
- Provide ongoing individual, family, or group counseling for the patient and/or natural supports according to departmental procedures, funding source requirements, and using evidence/strength-based approaches.
- Document clinical activity according to policies of the department and in compliance with required timelines.
- Provide case management services as identified on the Individualized Treatment Plan, including linking patients and supports to community resources and other services, as appropriate, and coordinating Behavioral Health treatment services and intervention strategies with internal and/or external supports, as appropriate.
- Provide consultation to primary care and/or specialty care providers regarding patients’ diagnosis, treatment plan, and status changes, as appropriate.
- Be readily available for same-day consults and assist in the wide range of patients that may be referred from Primary Care for behavioral health intervention. This may include behavioral health disorders, as well as prevention, adverse health behaviors, and chronic medical conditions.
- Conduct initial focused (brief) assessments and develop treatment plans based on the health care goals of the PCP and patient, within the bounds of the PCBH model, and according to policies of the department and funding source requirements.
- Provide immediate feedback to the PCP about the patient’s symptoms and functional impairments and develop a behavioral health change plan with recommendations to the PCP and team members regarding how they may support behavioral change plans for the patient in the future.
