Outpatient Licensed Practical Nurse - LPN LVN

Job Overview

Provide personal attention, kindness, and professional insight to patients while serving as a key member of the hemodialysis health care team. Work under the direct supervision of a registered nurse, contributing to safe and effective dialysis therapy.

• Deliver direct patient care for assigned patients, performing all technical aspects of dialysis procedures as prescribed.
• Administer and monitor treatments, document dialysis parameters and patient care information on flow sheets and individual patient records.
• Report significant changes in patient condition or equipment problems to the supervisor in a timely manner.
• Educate patients and families on end‑stage renal disease, dialysis therapy, diet, and medications.
• Assist with emergency procedures, including IV medication administration, CPR initiation, and response to dialysis‑related emergencies.
• Operate dialysis equipment safely, perform minor troubleshooting, and maintain a clean, sanitary treatment environment.
• Ensure compliance with Bloodborne Pathogen Control Policies and other organizational safety protocols.
• Participate in staff meetings, patient care plan meetings, and in‑service classes, sharing knowledge with colleagues.
• Coordinate care plans with Physicians, Dietitians, and Social Workers.
• Cover shifts at other area facilities as needed and support short‑staffing situations.

• Graduate of an accredited School of Practical Nursing (LPN/LVN) with current state licensure.
• One year of medical‑surgical nursing experience preferred; hemodialysis experience preferred but not required.
• Completion of a training course in the theory and practice of hemodialysis and CPR certification.
• Must complete the Nurses Technical Training Program upon hire or shortly thereafter.
• Ability to pass Ishihara's Color Blindness test as a condition of employment.
• Strong verbal and written communication skills.
• Ability to lift up to 30 lbs. and, with assistance, move equipment as required.

The role involves desk work, patient interaction, and periodic heavy lifting of patients and equipment. The employee must stand for prolonged periods, bend over, and may need to lift up to 200 lbs. with assistance. Travel to regional meetings and various facility locations may be required.
